How a family joins Forms.

A Cognia-accredited private school for student-athletes in grades 1 through 12. Rigorous academics and elite daily soccer development, integrated into one school day, on one campus, with one team of educators and coaches. Two North Texas locations. A development pathway to Madrid. Approved for Texas Education Freedom Accounts.

How admission works.

Selective, and mutual. We are assessing fit as much as you are.

Step 01
Visit, or talk to us.
Tour a campus, meet the coaching and academic staff, and watch part of a school day. If you would rather talk first, the inquiry form reaches admissions directly and is answered within one business day. Neither costs anything.
Step 02
Apply and be evaluated.
The application takes 15 to 20 minutes. Your athlete trains with our coaches and we look at academic readiness alongside it. Places are prioritized by fit and readiness, not by the date you applied.
Step 03
Enroll and plan the year.
Acceptance, enrollment paperwork, and transition planning with your current school and club. Eligible Texas families set up their Education Freedom Account at this stage.

A school built around development.

Forms Residency is the day school of Forms Academy, a youth development institution operating in Dallas-Fort Worth and Madrid. The Residency exists for families who want their child to receive a serious academic education and serious daily soccer development without sacrificing either. It is not a sports camp with classes attached. It is a Cognia-accredited school with elite training built into the day.

The school day starts and ends with training. Morning training, then four hours of focused academic instruction, then afternoon training. The same coaches, teachers, and staff see your child every day. There is no commuting between school, gym, and practice. Everything happens in one purpose-built environment. The structure gives families back what the traditional youth-athlete schedule takes away: time. Time to recover, time to do homework with support, time to be present at dinner.

Forms serves families who treat their child's development as a long-term project rather than a single-year experiment. Most of our students are serious about soccer at a competitive level, and many train alongside the FC Dallas Academy pathway through our partnership. But Forms is not exclusively for kids on a professional track. The methodology and the academic rigor benefit any student-athlete committed to growth on and off the field.

Forms Academy Residency Madrid is a satellite campus of our Cognia-accredited institution, embedded in Spain's elite youth football ecosystem. Every Residency day is structured the same way. Training bookends academics. Recovery is built in. Students arrive at 8 AM and leave at 4 PM, ready for family time and rest.

Forms Residency is a TEFA Approved Vendor.

Texas families approved for an Education Freedom Account can apply their funds directly toward Forms Residency tuition. For many families this substantially offsets the out-of-pocket cost. Eligibility, award amounts, and application timing are governed by the state program.

Daily instruction follows the Texas Essential Knowledge and Skills standards, ensuring academic alignment with Texas state expectations for grade-level mastery.

Forms Residency is an approved vendor for the Texas Education Freedom Account program, allowing eligible families to apply state funding toward tuition.

FC Dallas Development Partner

A strategic development partnership with FC Dallas creates a structured pathway from Forms training into the FC Dallas Academy elite player platforms, with shared evaluation by a joint technical committee.

Questions families ask.

Is the curriculum in-person or online?
Hybrid. Core instruction is delivered in person by certified educators on campus. Grade-specific coursework is supplemented through a flexible online platform that allows for personalization and independent pacing. Both happen inside the four hours of daily academic time. Students are supervised throughout. The online component is not remote learning. It is a structured tool used inside a fully attended school day.
How is academic progress measured?
Daily assessment through certified educators, plus triannual nationally normed standardized testing. Families receive transparent reporting on academic standing throughout the year. The Residency uses the same proficiency benchmarks expected of Texas grade-level mastery, with personalized intervention when a student is ahead or behind.

Primary Residency (grades 1st–6th) is at Z-Plex Sports Complex, 4220 E. Melissa Road, Melissa, Texas. Upper Residency (grades 7th–12th) is at INPOWERiQ Performance Center, 6051 Alma Road, McKinney, Texas, beginning the 2026-2027 academic year. How much does it cost and what's included?
Primary Residency is $1,375 per month and Upper Residency is $2,050 per month. Both add a one-time $45 application fee and a $495 annual supply and tech fee. Both rates cover full-day accredited academic instruction, two daily training sessions, performance and recovery services, and academic oversight. Families provide laptops, training uniforms, and lunch.
Do you qualify for Texas Education Freedom Account funding?
Yes. Forms Residency is an approved vendor for the Texas Education Freedom Account program. Eligible Texas families can apply their TEFA funds directly toward tuition. For many families the program substantially offsets the out-of-pocket cost. We can walk through eligibility specifics during your campus visit. Details are at formsresidency.org/tefa.
Is this only for elite-level soccer players?
No. The Residency serves student-athletes who are committed to growth, not players who have already reached a particular level. Many Residency students are tracking toward the FC Dallas Academy pathway. Others are committed soccer players whose families value the integration of serious academics and serious daily training. The methodology benefits any student-athlete willing to invest the time.
Can my child still play for their club team?
In most cases, yes. Many Residency students play for outside club teams or AlphaForms Academy teams. Coordination between Residency training and outside team schedules is something we work through case by case during the admissions conversation. The goal is to support each player's development pathway, not to lock them into a closed system.
What does the FC Dallas partnership mean for my child?
Forms Academy is an official strategic development partner for the FC Dallas Academy pathways. Players in the Forms system are tracked by a joint technical committee with FC Dallas Academy staff, evaluated continuously, and identified for entry into FC Dallas's elite player platforms when they demonstrate readiness. The partnership replaces single-tryout evaluation with multi-year development data. It is particularly significant on the girls' side, where Forms is the preferred development partner for the FC Dallas Girls Academy.
How does college recruiting work?
The Upper Residency curriculum is structured to meet NCAA initial-eligibility academic requirements. Game film analysis is part of every Upper Residency training week, which is a primary tool college recruiters use to evaluate players. Forms coaches actively support recruitment conversations for Residency students. The combination of strong academics, accumulated game film, and competitive game environments through AlphaForms and FC Dallas partnership exposure positions students well for college recruitment.
